What is an SSS E1 Form?
SSS stands for Social Security System under the Republic of the Philippines. It is a system to maintain a society with safety, security, and justice for members. It is a form of an identity system that has details of its registered members. It is used to justify your identification in various walks of life.
The SSS E1 form, popularly known as the E-1 form is used to apply for the Social Security System’s membership which allocates a unique Social Security Number to all its members. It consists of personal records of the members of the Social Security System.
Information required on an SSS E1 Form
From general personal details to specific ones, the form asks for all the details that might be required on an identity document. Basic information such as the name, address, contact number, and date of birth of the applicant is crucial. Along with which their profession or business and beneficiaries or dependents on the applicant are to be filled in on the E1 form.
How to Fill Out an Application for Issuance of a Social Security Number?
To fill out an application, one must first get an online or offline copy of the E1 form. An online copy can be obtained from the authorized website whereas the offline copy can be obtained from the nearest Social Security System office.
Nowadays, you can go on their website and fill out your details rather than print the form out. Digitally filled documents and e-signatures have become legally permissible.
You can fill the E1 form for yourself or on behalf of your beneficiary. The form has two parts, part I and part II. Part I is to be filled if you are applying for a Social Security Number for yourself. Part II is to be filled if you are applying for a Social Security Number for a dependent/beneficiary.
- To start with, download the E1 form from the authorized website. Start with section ‘A’ which asks for personal details. Fill out your full name, date of birth, sex, civil status, TIN (Tax Identification Number), nationality, religion, and detailed place of birth.
- After which you will have to fill in your contact details like detailed home address, contact number, email address, alternate/home phone number, and so on. You will also be required to fill in your parents’ full names.
- Moving on to section ‘B’, you will need to specify beneficiary details if any, and their date of birth.
- Section ‘C’ is only for self-employed individuals, OFW (Overseas Filipino Workers), or non-working spouses.
- For the self-employed individuals, you will be required to state your business/ profession, year of its start, and monthly earnings from it.
- For the OFW (Overseas Filipino Worker), you will have to mention your address overseas and monthly earnings.
- As a non-working spouse, you will need to state the Social Security/ Common Reference Number of your working spouse and their monthly earnings along with their name and signature.
- Section ‘D’ is the certification section that will require the applicant to state the truthfulness of the information mentioned by them in the form. Their signature and fingerprints will complete the form.
- The form also has an additional sheet that is to be filled by the applicant if applying for an SS Number on behalf of a dependent/ beneficiary.

The SSS authority has made the membership application process easier. They communicate through email and help you with the status of your application. If your membership application gets accepted, you can even get your Social Security Number via email.
It is always advised to get two copies of the form, one for submitting to the SSS authorities and one to be safely kept with the individual.
What is the SSS E1 Form Used for?
An E1 Form is used to apply for the issuance of a Social Security Number which registers the applicant as a member of the SSS, once approved. At times, it is also filled by an employer for a newly appointed employee as it contains personal records of the individual.
If you already are a member of the Social Security System but you have lost your original copy of the SSS E1 form or if you need it for something important, you can apply on the SSS website for the same. It takes a few minutes to apply for your originally filled out E1 form but bear in mind that it may take several months for you to receive the copy even via email.
